In order to accept your referral, we need to receive a Referral Form. (click on link).  If faxing is not possible, the referral information can be given verbally through the phone.  In addition to the referral form we need information regarding the person's current danger to him or herself as well as to others.  For this purpose the Statement of Current Status (click on link) needs to be filled out and signed. People with suicidal thoughts who are willing to contract for safety will, for this reason, not be denied acceptance.  The Statement of Current Status can be signed by a medical or osteopathic doctor, psychologist, licensed social worker, mental health worker, or nurse.  A Medical Clearance (click on link) is only necessary if the client is suffering from, or is at risk of any significant medical problem. Medical clearance can only be given by a medical or osteopathic doctor. Click here to download the Referral Cover Letter.

If you had difficulty downloading the form, please give a call. We are happy to fax them to you.
